Providence Health & Services Clinic Navigator in Portland, Oregon
Providence is calling a Clinic Navigator (0.6 FTE, Days) to Providence ElderPlace Cully in Portland, OR.
In this position, you will:
Be responsible for flow of patients seen in the Program of All-inclusive Care for the Elderly (PACE) centers.
Coordinate room set-up and clean-up.
Assist with obtaining vital signs, capillary blood glucose (CBG) results and other information needed for patient visits.
Complete lab requests, assist with the personal care of patients, and document care given.
Required qualifications for this position include:
High school graduate or equivalent
Current cardiopulmonary resuscitation (CPR) certification
Current Standard First Aid
American Heart Association Basic Life Support (AHA-BLS) certification must be obtained within six months of hire. Current caregivers have until 2019 to obtain AHA certification, which then must be maintained
Medical terminology training or equivalent work experience
2 years of experience working with, or caring for, a frail or elderly population in a professional setting
In Compliance with the Department of Human Services (DHS), criminal background check is required for this position, based on assigned duties. Fingerprinting also may be required
Preferred qualifications for this position include:
- Certified Nurse Assistant (CNA) certification
