Providence Health & Services RN Care Manager - PACE in Portland, Oregon
Providence is calling a RN Care Manager - PACE (1.0 FTE, Days) to Providence ElderPlace Laurelhurst in Portland, OR.
In this position, you will:
Provide care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues.
Perform population-based disease management approaches and the application of clinical protocols which are integral to the program.
Be a core member of the patient centered PACE interdisciplinary team (IDT).
Have goals that are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality.
Assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient’s functional status and independence.
Be responsible for the planning, overseeing and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT.
Be required to travel between multiple locations.
Required qualifications for this position include:
Current Oregon Registered Nurse (RN) license
2 years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience
1 year of experience with frail elderly population
Current Basic Life Support (BLS) at time of hire
American Heart Association Basic Life Support (AHA-BLS) for Healthcare Provider certification required within 6 months of hire
In compliance with company policy and state regulations, a Department of Human Services (DHS) a criminal background check may be required based upon the location of the position. Fingerprinting may be required.
Preferred qualifications for this position include:
Bachelor of Science in Nursing
Current recognition by a national professional organization in care management, geriatrics or hospice
