Providence Health & Services RN - PACE Care Manager in Portland, Oregon


Providence is calling a Registered Nurse - PACE Care Manager (1.0 FTE, Days) to Providence ElderPlace Irvington Village in Portland, OR.

In this position, you will:

  • Provide care management, care coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues.

  • Provide population-based disease management approaches and application of clinical protocols that are integral to the program.

  • Be a core member of the patient centered PACE (Program of All-inclusive Care for the Elderly) interdisciplinary team (IDT).

  • Meet the goals of care management to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality.

  • Assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient’s functional status and independence.

  • Be responsible for the planning, overseeing, and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT.

  • Travel between multiple locations.


Required qualifications for this position include:

  • Current state of Oregon RN licensure

  • Current Basic Life Support (BLS) at time of hire

  • American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification within 6 months of hire

  • 2 years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience

  • 1 year experience with frail elderly population

  • In compliance with company policy and state regulations, a Department of Human Services (DHS) a criminal background check may be required based upon the location of the position. Fingerprinting may be required.

Preferred qualifications for this position include:

  • Bachelor’s degree in nursing

  • Current recognition by a national professional organization in care management, geriatrics, or hospice
