Providence Health & Services RN - Wound Care Specialist in Tukwila, Washington
Providence is calling a Registered Nurse (RN) to specialize in Wound Care for Home Health (Day Shift, 1.0 FTE) at Providence Home Services of King County in Tukwila, Washington .

In this position you will:
Serve as a clinical resource to agency staff on issues pertaining to patients with wounds, pressure ulcers, ostomies (if area of expertise), fistulas and incontinence (if area of expertise). Makes joint visits for evaluation and education purposes as needed.
Perform comprehensive assessments, develop plans of care, coordinate care with other disciplines and provide direct patient care in the implementation of outcome oriented interventions under the physician ordered Plan of Care.
Perform OASIS comprehensive assessment primarily for patients with wounds and ostomies.
Promote and deliver coordinated wound, ostomy, and continence services per certification held.
Promote comprehensive care for patients through coordination of medical, agency, and community resources. Assists patients with preparation for discharge including supplies needed.
Collaborate with all healthcare team members (internal & external) toward the best outcomes for the patient and ministry.
Assess POC of ostomy patients (if area of expertise) and promotes self-care and psychosocial rehabilitation. Evaluates the effectiveness of care provided and revises care plan if indicated.
Serve as an educator to patients and their families, staff, physicians, and other healthcare professionals in the community regarding the management of ostomies, fistulas, wound, pressure ulcers, and incontinence (up to level of certification). This may include in-service education and orientation classes.
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Washington State RN License
Current Washington State Driver's License and acccess to daily use of insured, reliable vehicle
Current Basic Life Support for Healthcare Provider certification
2 years full time experience practicing as an RN
1 year recent (within last 3 years) clinical experience
Preferred qualifications for this position include:
Bachelor's Degree in Nursing
Graduate from an accredited WOCN program and have current WOCN license
Licensed as Wound Care Specialist or Enterostomal Therapist
Experience in a Medicare-certified Home Care or Hospice agency/facility
Experience as a Wound Specialist
